Registered Nurse
Part-time 20 hours Mon-Fri
West Trenton, NJ
Manage cases identified through the disease mngnt program: asthma, diabetes, and congestive heart failure, while ensuring the effective implementation of interventions, and efficient utilization of benefits. You will provide telephonic assessments/planning, coordinate the delivery of care, participate in QI activities, and utilize the case management process to set priorities, plan, organize, and implement interventions that are goal-directed toward self-care outcomes. You will also make site visits to physician's offices and clinics to promote the program and support quality care.
Requires a NJ RN license, min. 3-4 yrs of clinical experience, preferably in home care, discharge planning, case management, superior problem-solving abilities, along with excellent organizational, oral/written communication and PC skills. Managed care experience preferred. Minimal travel is required.
